F. Right to a Paper Copy of This Notice You have the right to obtain a paper copy of this notice upon request, even if you have agreed to receive the notice electronically.

We reserve the right to change this Privacy Policy at any time. The revised policy will be effective for all PHI we maintain. We will post a copy of the current policy on our website, and the effective date will be indicated at the top of the policy.
